Process hazard analysis (PHA) is critical for identifying hazards and assessing risk in a process facility. While other books focus on the methods of PHA, Applied Process Hazard Analysis: The Operational Questions instead looks at various types of industrial equipment and their failure modes and provides questions facilitators should ask for each process. For each type of equipment, the author discusses the modes of failure, including not only common failure modes but also special or unusual types of failures drawn from actual situations. Examples and illustrations are given, where appropriate. Once the failure modes are described, a series of questions is offered. The PHA facilitator is encouraged to ask these specific questions of the PHA team in order to stimulate brainstorming among the PHA team members. The goal is to help identify previously unrecognized hazards in the process under review.
